Description

Players interested in participating in the H3 Elite League must complete this player registration, signing the waiver, and submitting their league fee.

League Fee: $100 per player

Players have 2 payments options:

Pay in Full: $100 at registration
Two Payments: $50 at registration and $50 due by April 5th.

Players who choose the installment option must complete the second payment by April 5th to remain eligible to play.

Only fully registered and paid players will be allowed to participate in league games.

Important: Players who have note completed full payment before the deadline will be removed from the active roster until payment is completed.

H3 Elite League – Participant Waiver and Informed Consent

By registering for and participating in activities organized by H3 Elite League, I acknowledge and agree to the following:

1. Acknowledgment of Risk
I understand that participation in sports activities, including basketball and related events, involves inherent risks of injury, including but not limited to sprains, fractures, head injuries, or other physical harm. I voluntarily choose to participate and accept all associated risks.

2. Medical Fitness
I certify that I am physically capable of participating in athletic activities and that I have not been advised otherwise by a medical professional. I understand it is my responsibility to consult a physician if I have concerns about my health.

3. Release of Liability
I hereby release, waive, and discharge H3 Elite League, its organizers, staff, volunteers, sponsors, facility owners, and partners from any and all liability, claims, demands, or causes of action arising from injury, illness, or damages that may occur as a result of participation in league activities.

4. Emergency Medical Authorization
In the event of an injury or medical emergency, I authorize league organizers to seek medical treatment on my behalf if I am unable to do so. I understand that I am responsible for any medical costs incurred.

5. Code of Conduct
I agree to follow all league rules, demonstrate sportsmanship, and respect officials, players, staff, and spectators. I understand that failure to comply may result in suspension or removal from the league without refund.

6. Media Release
I grant permission to H3 Elite League to photograph or record video during events and use such media for promotional or marketing purposes, including on websites and social media platforms.

7. Agreement
By checking the box or signing below, I confirm that I have read, understood, and voluntarily agree to the terms of this waiver and informed consent.
